Director of Patient Services
Job Summary
Under the direction of a physician and supervision of the Director of Patient Services, the Hospice Social Worker conducts client and family assessments and provides direct casework service and counseling to clients and families. S/he functions as an integral member of the interdisciplinary team. S/he initiates and maintains contacts with all community agencies, conducts bereavement assessment, and participates in the implementation and facilitation of support groups.
Qualifications
- Graduate of a Bachelor's program in social work accredited by the Council on Social Work Education
- Minimum of one year experience in health care
- Hospice experience preferred
- Understands Hospice philosophy and issues related to death and dying
- Current state of Ohio licensure.
